The deadline to request an absentee ballot by mail is October 29th. Alternatively, you still mat apply in person at the Suffolk County Board of Elections up until November 4th, 2019.
All absentee ballots must be either postmarked prior to November 4th or hand delivered to the Board of Elections by Tuesday, November 5th. All mailed-in ballots must be reveiced by the Board of Elections by November 12th.
In filing out the form be sure to do the following:
Line 1, Check - "absence from county" or applicable option.
Line 2, Check "general election only".
Line 3, Full last name, first name, middle initial, and suffix with which you are registered.
Line 4, Your date of birth and residential county.
Line 5, The residential address where you are registered (No PO Box number).
Line 6- Leave blank.
Line 7, Check- "mail to me" and fill In applicable address.
Sign and date (Ignore the request below your signature for witness to mark, etc.)
Mail the form to Suffolk Board of Elections, P.O. Box 700, Yaphank, NY 11980
NOTE- Election laws prohibit the post office from forwarding the ballot. Please provide appropriate delivery address.
